چکیده

Death is a rare but devastating complication of chronic total occlusion (CTO) percutaneous coronary intervention (PCI). We examined the clinical characteristics and procedural outcomes patients who died periprocedurally in Prospective Global Registry for Study CTO Intervention (PROGRESS-CTO, NCT02061436). Of 12,928 underwent PCI between 2012 2022, 52 (0.4%) during index hospitalization. Patients were more likely to have history heart failure (43% vs. 28%, p=0.023). The J-CTO (2.8 ± 1.1 vs 2.4 1.3, p=0.019), PROGRESS-CTO mortality (2.6 0.9 1.6 1.1, p<0.001), pericardiocentesis (2.9 1.9 p<0.001) scores higher died. In these patients, use left ventricular assist devices was (41% 3.5%, p<0.00), retrograde crossing often first strategy (33% 13%, p<0.001). cause death cardiac 43 (83%) non-cardiac 9 (17%). Complications leading were: tamponade 30 (58%), acute myocardial infarction (17.3%), arrest/shock 4 (7.7%). Non-cardiac causes stroke 3 (5.8%), renal 2 (3.8%), respiratory distress hemorrhagic shock (3.8%) (Figure). Approximately 0.4% main 58%. might help risk stratification planning undergoing PCI.

Chronic total coronary occlusion percutaneous intervention.

Chronic total coronary occlusions (CTO) are encountered frequently, found in up to 50% of patients with coronary artery disease (CAD) referred for elective angiography. Treatment strategy for an individual patient is often changed on the basis of the presence or absence of a CTO (1). Percutaneous coronary intervention (PCI) of CTO remains challenging, with observed success rates that are signif...

Chronic total occlusion percutaneous coronary intervention in 2014 beyond the J-CTO score (Japanese Multicenter CTO Registry): chance favors the prepared mind.

SEE PAGE 257 P ercutaneous coronary intervention of chronic total occlusions (CTOs) represents the most technically challenging procedure in contemporary interventional cardiology (1,2). Although large, definitive randomized trials are lacking, a growing body of evidence suggests that successful percutaneous CTO revascularization relieves symptoms, improves left ventricular systolic function, r...

Development and Validation of a Scoring System for Predicting Periprocedural Complications During Percutaneous Coronary Interventions of Chronic Total Occlusions: The Prospective Global Registry for the Study of Chronic Total Occlusion Intervention (PROGRESS CTO) Complications Score

BACKGROUND High success rates are achievable for chronic total occlusion (CTO) percutaneous coronary intervention (PCI) using the hybrid approach, but periprocedural complications remain of concern. Although scores estimating success and efficiency in CTO PCI have been developed, there is currently no available score for estimation of the risk for periprocedural complications. We sought to deve...

Impact of Hemodialysis on Procedural Outcomes of Percutaneous Coronary Intervention for Chronic Total Occlusion: Insights From the Japanese Multicenter Registry

BACKGROUND Among patients treated with percutaneous coronary intervention for chronic total occlusion (CTO-PCI), patients on long-term hemodialysis are at significantly high risk for cardiovascular mortality and morbidity. However, clinical or angiographic predictors that might aid in better patient selection remain unclear. We aimed to assess the acute impact of hemodialysis in patients who un...

Use of Intravascular Imaging During Chronic Total Occlusion Percutaneous Coronary Intervention: Insights From a Contemporary Multicenter Registry

BACKGROUND Intravascular imaging can facilitate chronic total occlusion (CTO) percutaneous coronary intervention. METHODS AND RESULTS We examined the frequency of use and outcomes of intravascular imaging among 619 CTO percutaneous coronary interventions performed between 2012 and 2015 at 7 US centers. Mean age was 65.4±10 years and 85% of the patients were men. Intravascular imaging was used...
